The primary objective of this position is to facilitate direct access to primary care services for patients, while fostering a well-rounded, skilled clinical team to enhance overall patient care.
Key Responsibilities
As the initial point of contact for patients seeking primary care for specific conditions, the practitioner will conduct thorough health assessments based on advanced nursing knowledge and skills. Responsibilities include providing guidance, initiating treatment, and referring patients to the on-call GP as necessary.
Clinical Duties:
- Prescribe medications tailored to the needs of the practice population.
- Conduct minor illness clinics to facilitate early diagnosis and appropriate referrals for further intervention.
- Collaborate with general practitioners, practice nurses, and other healthcare professionals to develop protocols, standards, and audit tools that ensure effective care delivery.
- Identify, assess, and diagnose patients with intricate health needs across the practice population.
- Lead the implementation of specialized nursing treatments for patients with complex healthcare requirements.
- Develop initiatives aimed at improving health and well-being, addressing the specific needs of communities and individuals.
- Work collaboratively to influence policies that enhance health outcomes and reduce disparities.
- Drive the application and evaluation of evidence-based practices to elevate the services provided by the nursing team.
Chronic Disease Management:

Special Requirements:
- Registered General Nurse
- First level degree or equivalent
- Independent Prescriber V300 qualification
- Minimum three years of community nursing experience
