About the Role

We are seeking a skilled and compassionate Community Staff Nurse - Twilight to join our dedicated team at Surrey Downs Health and Care. As a key member of our team, you will play a vital role in assessing, developing, implementing, and evaluating comprehensive care packages for patients, respecting their privacy and dignity.

Main Responsibilities

  • Work collaboratively with the District Nurse to lead team initiatives and promote patient health and well-being.
  • Assess individual care needs, implement, and evaluate holistic programmes of care.
  • Develop effective communication and liaison systems, and maintain clear and comprehensive records according to SDHC procedures.
  • Facilitate/undertake training and professional updating for team and students, and act as an assessor and mentor to qualified and unqualified staff.
  • Maintain, develop, and monitor standards of evidence-based nursing care, and maintain own professional and clinical integrity in line with NMC guidelines.
  • Support/mentor team members and participate in clinical supervision activities, and work without direct supervision and maintain close liaison with the senior team member, deputising as required.
