Job Summary

This role is part of the Dorset Clinical Commissioning Group's commitment to providing high-quality community healthcare services. As a MAS Specialist Community Practitioner, you will work closely with our team to deliver exceptional patient care and support.

Main Responsibilities

  • Deliver community-based healthcare services to patients in need, working collaboratively with other healthcare professionals to ensure comprehensive care.
  • Develop and maintain strong relationships with patients, families, and carers to understand their unique needs and preferences.
  • Conduct thorough assessments and develop personalized care plans to address patients' physical, emotional, and social needs.
  • Work effectively in a team environment, communicating clearly and respectfully with colleagues to ensure seamless care delivery.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices to provide evidence-based care.

Requirements

  • NVQ level 2 or 3 or equivalent level of experience in a healthcare setting.
  • Literacy and numeracy skills to effectively communicate with patients and colleagues.
  • Ability to work in a fast-paced environment, remaining calm and composed under pressure.
  • Basic IT keyboard skills to maintain accurate records and communicate with the team.
  • A level of fitness to participate in manual handling, PMVA, and Breakaway training.
  • Demonstrable skills in written and spoken English to effectively communicate with patients and colleagues.
