Position Summary: We are currently seeking a Lead Care Facilitator to join our team in a residential environment, focusing on assisting adults with both learning and physical challenges.

This is a permanent role offering between 35 to 42 hours of work per week. While prior experience in a similar capacity is crucial, holding an NVQ level 3 in Health and Social Care is not a requirement.

Key Responsibilities:

  • Maintain and update care plans and risk assessments to ensure they accurately reflect the needs of each individual.
  • Oversee and mentor a team of support staff, conducting supervisions and performance reviews as necessary.
  • Assist service users in accordance with their personalized support plans, respecting their preferences and needs.
  • Exhibit a strong commitment to the safeguarding of adults, promptly recognizing and reporting any signs of harm, risk, or neglect.
  • Encourage service users to lead active and healthy lifestyles.
  • Participate in training sessions, including orientation, team meetings, and individual supervisions.
  • Support service users in daily living activities, such as shopping, meal preparation, and household tasks.
  • Promote an active lifestyle for service users.
