About the Role
We are seeking a skilled and compassionate Registered Nurse to join our Community Nursing Team in Bideford. As a Community Nurse, you will play a vital role in providing high-quality care to individuals in their own homes or relevant care settings.
Key Responsibilities
- Deliver person-centered care to individuals with complex needs, promoting independence and supporting their well-being.
- Conduct thorough assessments, develop and implement care plans, and review progress to ensure optimal outcomes.
- Work collaboratively with the Clinical Nurse Specialist and other healthcare professionals to provide seamless care.
- Develop and maintain strong relationships with individuals, families, and carers to ensure effective communication and support.
- Stay up-to-date with best practices, policies, and procedures to provide evidence-based care.
Requirements
- Registered Nurse with a valid NMC registration.
- Experience in community nursing or a related field.
- Strong communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Flexibility to work varied shifts, including weekends and bank holidays.
