Nurse Care Coordinator

Highwood Care Home is seeking a dedicated Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for delivering high-quality care to our residents, while also coordinating with the care team to ensure seamless transitions and continuity of care.

Key Responsibilities:

  • Craft personalized care plans that meet the unique needs of each resident
  • Collaborate with residents, families, and healthcare professionals to ensure accurate and effective care delivery
  • Lead the care team to ensure timely and safe handovers, adhering to protocols and procedures
  • Monitor resident health and well-being, identifying areas for improvement and implementing changes as needed
  • Administer medications safely and in accordance with MAR sheets and protocols
  • Foster strong relationships with residents, families, and colleagues to ensure a supportive and respectful environment
  • Promote independence and autonomy in residents, while providing hands-on assistance as needed
  • Act on feedback from the care team to ensure resident health and well-being are always prioritized
  • Champion the dignity and individuality of every resident, advocating for their needs and preferences
  • Encourage social interactions and activities to enrich the lives of residents and create a vibrant community
  • Prioritize safety and maintain a vigilant eye on health and safety to ensure a secure environment
  • Support balanced nutrition and hydration, monitoring residents at risk of weight changes or dehydration
  • Respond to emergencies swiftly and effectively, utilizing the on-call system and reflective practices to continually improve our care

About You:

  • You must be a Registered General or Mental Health Nurse (RGN or RMN) with an active NMC Nursing PIN
  • Care home experience is preferred, but what really matters is your passion for care and dedication to improving the lives of our residents
  • You have a natural ability to connect with people, bringing warmth, understanding, and a calm, patient demeanor to every interaction
  • Communication is your superpower, whether you're talking with a resident about their needs, updating a family member, or guiding the care team
