Advanced Clinical Practitioner

We are seeking an experienced and highly motivated Advanced Clinical Practitioner to join our dynamic clinical team in Plymouth. As a key member of our practice, you will play a vital role in providing high-quality patient care and contributing to the delivery of our clinical services.
Main Responsibilities- Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
- Clinically examine and assess patient needs and plan clinical care accordingly.
- Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness.
- Work with patients to support compliance with and adherence to prescribed treatments.
- Provide information and advice on prescribed, or over-the-counter medication on medication regimes, side-effects, and interactions.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent, or emergency situations, including initiation of effective emergency care.
- Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
- Refer patients directly to other services/agencies as appropriate.

We use TPP SystmOne clinical system and are proud of our clinical and administrative teams and the level and continuity of care that they give.
Person Specification- Qualified Nurse
- MSc in nursing or working towards this
- Experience of working within a nursing and clinical team
- Experience of working in Primary Care and General Practice
