Clinical Case Manager

Job Title: Clinical Case Manager
Job Summary:
The Clinical Case Manager will play a critical role in enhancing overall student wellness by providing direct case management services, crisis intervention, clinical services, and support. This position will also provide consultation to the WPI community, preventative and educational outreach initiatives, and administrative support of the case management position.
Responsibilities:
- Provide case management and clinical outreach services to students, including creating and maintaining a referral database for mental health providers and assisting students in accessing services from community resources.
- Develop and implement systems for follow-up with students who have been referred to community resources and serve as a liaison between community mental health providers and WPI staff.
- Assist students who have been assessed at EMH or hospitalized for mental health reasons transition back to WPI by coordinating with the hospital for follow-up and discharge planning.
- Collaborate with counseling staff in treatment and disposition planning for students and maintain appropriate records and documentation of case management activities.
- Build liaison relationships with off-campus mental health providers, private practitioners, hospitals, and inpatient units, and other community partners.
- Support administrative and campus outreach initiatives, including attending and participating in regular on-call rotations, staff meetings, and leading case management meetings.
- Provide crisis assessment, intervention, and high-quality counseling to WPI students, including providing on-call and urgent counseling sessions and communicating with campus partners while on-call.
Requirements:
- Masters in Social Work, Counseling, Psychology, or other related field
- LCSW (LICSW eligible), LICSW, MA (LMHC eligible), LMHC, or equivalent
- 2 years' experience
