We are seeking a highly skilled and experienced professional to join our team as a Learning Disability Screening Practitioner - Health Equity Specialist. This is an exciting opportunity to make a meaningful impact on the health outcomes of people with learning disabilities.
Key Responsibilities- Develop and implement strategies to improve screening uptake among people with learning disabilities, working closely with system partners, community organizations, and individuals with lived experience.
- Conduct thorough assessments to identify barriers to access and develop targeted interventions to address these challenges.
- Provide clinical supervision and mentoring to junior staff, promoting a culture of excellence and continuous improvement.
- Collaborate with the Dorset Screening programme service to ensure alignment with the NHS Long Term Plan and priority commitments to support people with learning disabilities.
- Work closely with CTLD MDTs and Primary Care to facilitate access to national screening programs for patients with learning disabilities.
- Registered practitioner, public health professional, or lived experience peer worker with a degree or diploma level qualification supplemented by post-registration training.
- Demonstrable knowledge of assessment and health promotion interventions in the area of specialism, with experience at Practitioner Band 5 level or equivalent.
- Ability to prioritize and organize workload effectively, with strong communication and interpersonal skills.
