We are looking for a committed Senior Clinical Practitioner to become a vital part of our healthcare team, dedicated to providing outstanding quality care and formulating comprehensive treatment strategies for patients in their residences. As an integral member of our multidisciplinary team, the successful candidate will work in close collaboration with medical consultants, general practitioners, pharmacists, nursing staff, therapists, and healthcare support personnel.
This position necessitates a significant degree of independence and intricate decision-making, underpinned by a master's level qualification that includes clinical practice, leadership, management, education, and research. The practitioner will oversee clinical care in partnership with patients, families, and caregivers, evaluating complex issues to create innovative solutions that improve outcomes and experiences.
Key Responsibilities
- Deliver advanced clinical practice independently in areas such as general medicine, frailty, respiratory care, and cardiology, with an emphasis on supporting patients as an alternative to hospital admission.
- Collaborate with colleagues and management to integrate care pathways and patient management across both community and acute care settings.
- Provide consistent, high-quality, person-centered care through advanced clinical judgment and critical thinking, in alignment with the National Framework for Advanced Clinical Practice, while embracing the four pillars of clinical practice: leadership and management, education, and research.
- Exhibit advanced theoretical and practical knowledge across various specialist fields within community services.
- Operate autonomously, demonstrating a high level of professional independence, making critical decisions, and offering guidance where established protocols may not exist.
- Engage in multidisciplinary teamwork, applying clinical judgment and independent decision-making.
- Work collaboratively with patients, families, and caregivers, employing a variety of assessment methods, including history-taking, holistic assessments, and health needs evaluations.
- Demonstrate effective communication skills, assisting individuals in decision-making, care planning, and promoting positive changes, utilizing frameworks that encourage person-centered approaches in health and care.
- Utilize expertise and decision-making capabilities to apply clinical reasoning in complex scenarios, synthesizing information from diverse sources for evidence-based judgments and diagnoses.
- Initiate, assess, and adjust a range of interventions, which may include prescribing medications, therapies, lifestyle recommendations, and care plans.
- Exercise professional judgment to manage risks effectively in complex and unpredictable situations.
- Conduct joint visits with multidisciplinary team members to promote holistic, person-centered assessments and care approaches.
- Maintain a continuous awareness of the importance of respecting individuals' privacy, dignity, and uniqueness, aiming to provide care in an environment that aligns with the preferences and current physical and emotional needs of the person.
- Continuously develop and review acute illness pathways and practices to ensure efficient and effective service delivery, providing optimal value to individuals and organizations.
- Practice in accordance with the relevant professional conduct codes and within their scope of practice, being accountable for their decisions, actions, and omissions at this level of practice.
