Job Title: Clinical Pharmacist - Primary Care Network

We are seeking a highly motivated and experienced Clinical Pharmacist to join our Primary Care Network team. As a Clinical Pharmacist, you will play a key role in providing high-quality patient care and contributing to the development of our network.

Key Responsibilities:
  • Provide clinical pharmacy services to patients in primary care settings, including medication reviews, prescribing, and patient education.
  • Work closely with GPs, nurses, and other healthcare professionals to ensure seamless care coordination and patient outcomes.
  • Develop and implement clinical pharmacy services to improve patient care and reduce healthcare costs.
  • Contribute to the development of our network's clinical governance and quality improvement initiatives.
  • Collaborate with other healthcare professionals to identify and address health inequalities and improve patient outcomes.
Requirements:
  • Registration with the General Pharmaceutical Council (GPhC) as a pharmacist.
  • Experience in primary care pharmacy or a related field.
  • Strong clinical skills and knowledge of pharmacy practice.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
