About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Complex Community Nurse. As a key member of our community care team, you will be responsible for assessing and reviewing individual care packages, providing practical training to care workers, and clinical supervisions and instructions using your clinical expertise and guidance.

You will work closely with our healthcare workers at the point of care, which may include home, school, community, and day centres. Your clinical knowledge and experience will be essential in interacting with ICBs, Case Management, Social Services, private and NHS hospitals, nursing homes, and learning disability establishments.

As a Complex Community Nurse, you will have the opportunity to develop your skills through self-directed learning, coaching conversations, e-learning modules, and access to further qualifications and courses. You will be supported to develop your career while empowering you to feel proud of the impactful and meaningful work that you do.

Key Responsibilities

Assess and review individual care packages to ensure the best possible outcomes for our clients

Provide practical training to care workers to enhance their skills and knowledge

Conduct clinical supervisions and instructions to ensure high-quality care

Work closely with our healthcare workers at the point of care to deliver exceptional care

Interact with ICBs, Case Management, Social Services, private and NHS hospitals, nursing homes, and learning disability establishments to ensure seamless care delivery
