We are seeking an experienced and skilled Associate Nurse to join our Cardiology team at Guy's and St Thomas' NHS Foundation Trust. As an Associate Nurse, you will play a vital role in providing high-quality care to patients with cardiac conditions.
Job Summary:The successful candidate will be responsible for managing the ongoing clinical management of patients with cardiac conditions, working closely with the Consultant Nurse in Cardiology to ensure seamless care. You will be expected to deliver a high standard of care, utilizing your expertise and analytical skills to assess, plan, implement, and evaluate patient care.
Key Responsibilities:- Manage outpatient follow-up care for patients who have undergone PCI, including assessment, planning, implementation, and evaluation of care.
- Use expert judgment and analytical skills to recognize changes in patient condition, referring to senior/medical staff as necessary.
- Ensure accuracy of documentation and maintain an efficient and accurate record system with relevant information in agreement with the cardiology team.
- Be responsible for data entry and maintaining the BCIS/CCAD patient database.
- Assist with the running of pre-admission clinics for all elective cardiology admissions.
- Manage psychological issues related to caring for patients on a long-term basis.
- Assist with the running of other cardiology nurse-led services.
- Act as an expert resource for patients/relatives/colleagues relating to the care of their caseload of patients.
- Maintain patient confidentiality, privacy, dignity, religious, and cultural needs at all times.
- Be an independent non-medical prescriber.
- Work within the framework of the Trust Non-Medical Prescribing Protocol.
- Maintain an environment for safe and effective patient care in accordance with the Trust Risk Management Policy.
- Work effectively within the Trust's adult multidisciplinary cardiology team as required, ensuring an individualized, evidence-based optimum service for each patient and their family.
- Act as an advocate for cardiology patients, enabling them to express their needs effectively when dealing with the multidisciplinary team.
- Promote collaborative relationships with other healthcare professionals for the benefit of quality patient care.
- Facilitate a seamless service between the Trust and locally based services by ensuring good ongoing communication between tertiary, secondary, and primary care.
- Deputize for the Consultant Nurse in Cardiology when required.
