About the Role

We are seeking a highly skilled and experienced Clinical Pharmacist to join our Clinical Adult Mental Health team at the Whiteleaf Centre in Aylesbury. As a specialist mental health pharmacist, you will play a key role in supporting patients, carers, and healthcare professionals with medication issues.

Main Responsibilities

  • Develop excellent clinical relationships with patients, carers, and healthcare professionals.
  • Support patients, prescribers, and other healthcare professionals through attendance at multidisciplinary team meetings, medication reviews, and discharge planning.
  • Ensure the correct application of national and local guidance for therapies involving medicines.
