About the Role

We are seeking a highly skilled and experienced Clinical Specialist Pharmacist to join our team at Buckinghamshire Healthcare NHS Trust. As a key member of our clinical pharmacy team, you will play a vital role in delivering high-quality pharmaceutical care to our patients.

Key Responsibilities
  • Deliver a specialist pharmacy service to different care groups within the trust, providing expert advice and guidance to healthcare professionals.
  • Identify and prioritize high-risk patients requiring specialist pharmacist input, delivering proactive and prompt pharmaceutical advice to ensure optimal patient outcomes.
  • Undertake medicines management duties, including medicines reconciliation, optimization, and improving prescribing practices to support timely discharge.
  • Provide specialist and accurate information and educational training to medical and nursing staff, as well as patients, regarding medications, therapeutic effects, monitoring, and potential adverse effects.
  • Participate in the provision of a sessional dispensary service for inpatients and outpatients at any pharmacy department within the trust, adhering to agreed trust and national standards.
  • Contribute to guideline development, medicines safety, drug budget management, and formulary applications to ensure the highest standards of patient care.
