Part-Time Practice Nurse

Purosearch Ltd is seeking a dedicated and compassionate Practice Nurse to join their dynamic and patient-centered team in Crawley, West Sussex.
Key Responsibilities- Patient Care: Deliver a comprehensive range of nursing services, including chronic disease management, immunizations, wound care, and cervical screening. The successful candidate will work closely with patients to provide tailored care that meets their individual needs.
- Health Assessments: Conduct thorough health assessments, develop and implement individualized care plans, and provide ongoing monitoring and support to patients with long-term conditions such as diabetes, asthma, and hypertension.
- Team Collaboration: Work collaboratively with GPs, healthcare assistants, and other members of the multidisciplinary team to ensure a holistic approach to patient care.
- Health Promotion: Lead and participate in health promotion activities, patient education programs, and community outreach efforts aimed at improving public health outcomes. The role includes organizing and delivering clinics on topics such as smoking cessation, weight management, and lifestyle advice.
- Maintain meticulous and up-to-date patient records using the practice's electronic health records (EHR) system.
- Ensure that all documentation meets the required legal and professional standards, and that patient confidentiality is strictly maintained at all times.
- Contribute to the development, review, and implementation of practice protocols, policies, and procedures.
- Registered Nurse: Hold a valid NMC registration with evidence of continuous professional development.
- Experience: Previous experience in a primary care or practice nursing role is essential.
- Clinical Skills: Strong clinical skills, particularly in the management of chronic diseases, wound care, and health promotion.
- Communication Skills: Excellent verbal and written communication skills are required, with the ability to engage effectively with patients, colleagues, and external healthcare professionals.
