Position Overview:

The Care Transition Specialist will play a crucial role within the Transfer of Care Hub, assisting with the efficient and timely triage of referrals for individuals discharged from acute care settings.

The successful candidate will collaborate with various professionals to triage assessments and coordinate placements for patients being discharged, ensuring they are directed to the most suitable environment for their needs.

Primary Responsibilities:

  • Facilitate the prioritization of care following senior clinical assessments and manage the referral processing within established guidelines.
  • Oversee a designated caseload of patients to ensure the delivery of a comprehensive Discharge to Assess Service, leading efforts to support community care through enablement and identifying suitable bed-based placements.
  • Act as an advocate for patients within the health and social care framework, ensuring safe and timely discharges while empowering patients and families to engage in self-care and utilize voluntary services.
  • Collaborate with professionals across acute, community, and partner organizations, including referrals from out-of-area acute and private hospital settings, to enhance access to community services for Oldham residents requiring ongoing support post-discharge.

Essential Qualifications:

  • A willingness to pursue QCF Level 2 in Health and Social Care, if not already obtained.
  • Experience working with multiple agencies, including health professionals, care management, and acute/primary care services.
  • Demonstrated knowledge and understanding of assessing functional abilities and formulating discharge plans.
  • Proven ability to manage situations calmly with a solution-oriented approach.
  • Proficient IT skills and experience working with various electronic systems and procedures, such as Mosaic and System 1.
  • Flexibility to work evenings and weekends as required.
