Job Title: Mental Health Practitioner

We are seeking a skilled Mental Health Practitioner to join our Crisis Resolution and Home Treatment Team in South West Essex. As a key member of our team, you will play a vital role in providing high-quality community care to individuals experiencing mental health crises.

Main Responsibilities:

  • Assess and support individuals in their own homes, providing intensive home treatment and care.
  • Work collaboratively with our community teams, liaison services, and partner organizations to provide a range of support options.
  • Act as a gatekeeper to all psychiatric admissions, supporting and managing patient flow through the urgent care system.
  • Develop and implement effective risk management strategies to ensure the safety and well-being of service users.
  • Contribute to the delivery of high-quality care through supervision of practice, clinical audit, evidence-based practice, teaching, and supporting professional colleagues.

Requirements:

  • Professional qualification in a relevant field (OT, Social Work, RMN).
  • Educated to degree level.
  • Good understanding of the Mental Capacity Act and its application in practice.
  • Good understanding of person-centered care and the Recovery Model.
