Main Responsibilities

  • Undertake health assessments, administer medication and treatments, and perform diagnostic tests.
  • Provide and evaluate a range of health interventions to ensure that patients' health and social care needs are met.

  • Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition.
  • Diagnose and manage both acute and chronic conditions, integrating drug- and nondrug-based treatment methods into a management plan.
  • Work with patients to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication, medication regimes, side effects, and interactions.
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

About Us

We are a rural General Medical Services (GMS) 9900 patient surgery with 4 Partners, 4 Salaried GPs, and a fantastic clinical and practice team with an on-site Dispensary. We are located in a modern, recently extended surgery with community nurses located on site as well as an X-ray suite and glorious surrounding countryside.

Person Specification

Qualifications

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
  • A registered prescriber.
  • Competence to undertake asthma, COPD, and diabetic clinics and reviews, holding relevant certificates.

Experience

  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations, and immunizations, assisting with minor surgery, dressing, and wound care, and recording spirometry.
  • A compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives, and carers, as well as their colleagues at the practice.
  • Knowledge and experience of its application in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
