- Complete tasks to facilitate safe and successful discharges from hospital, including property inspections, collecting belongings, and arranging necessary services.
- Provide regular visits and support to individuals in the community who require additional assistance, ensuring their homes are warm, safe, and well-maintained.
- Excellent communication and interpersonal skills, with the ability to work effectively with vulnerable and older adults.
- Aptitude for working under pressure and managing daily tasks efficiently.
