In this role, you will deliver motivational outreach interventions to individuals who are dependent and entrenched, and who may be hesitant to engage with traditional services.
You will also manage and coordinate the Trust's Naloxone Peer to Peer initiative across the East Kent area.Your contributions will be vital in achieving the objectives set by Forward Trust and local commissioners.
This position requires flexibility and creativity, which may involve traveling to various projects within the East Kent region to ensure continuity of care for our client group.
The following skills and experiences are desirable but not mandatory, as we provide a comprehensive induction and ongoing training program to enhance our workforce:
- Proven experience and enthusiasm for engaging individuals through proactive outreach
- Background in substance misuse services
- Experience in conducting thorough assessments, risk management plans, and developing SMART care plans
- Ability to communicate in multiple languages is a plus but not essential
