Job Summary

Old Road Medical Practice is seeking a skilled Practice Nurse to join their multi-disciplinary team. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of the practice.

Main Responsibilities

  • Conduct health assessments, administer medication, and perform diagnostic tests to ensure accurate diagnoses and effective treatment plans.
  • Provide and evaluate a range of health interventions to meet patients' health and social care needs.
  • Develop and implement individualized care plans to promote health and well-being, and manage long-term conditions in accordance with national and local policies.
  • Review medication for therapeutic effectiveness and work with patients to support adherence to prescribed treatments.
  • Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
  • Support and manage the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Implement and participate in vaccination and immunization programs for adults and children, including patients traveling abroad.
  • Meet the needs of patients presenting for opportunistic wound care.
  • Manage hypertension and run asthma, COPD, and diabetes clinics and reviews.
  • Conduct new patient and health checks.
