Job Summary

Main Responsibilities

  • Conduct comprehensive health assessments, administer medications and treatments, and perform diagnostic tests to ensure accurate diagnoses and effective patient care.
  • Develop and implement individualized health plans to address patients' complex needs, including long-term conditions, and provide education and support to promote healthy behaviors.
  • Collaborate with healthcare professionals to coordinate patient care, share knowledge, and improve health outcomes.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices to provide evidence-based care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives to enhance patient satisfaction, safety, and quality of care.

Person Specification

  • A relevant nursing degree and current NMC registration with an active PIN.
  • Experience in treatment room consultations, phlebotomy, ECG recording, screening, and management of long-term conditions.
  • Excellent communication and interpersonal skills, with a compassionate and empathetic approach to patient care.
  • Knowledge of current issues in nursing and the wider NHS, with a commitment to ongoing professional development.
