About the Role
We are seeking a highly motivated and compassionate Social Prescriber Link Worker to join our team at Mendip Vale Medical Group. As a key member of our multidisciplinary team, you will play a vital role in empowering patients to take control of their health and wellbeing.
Job Summary
The successful candidate will connect patients to community and statutory services that meet their practical, social, and emotional needs. You will develop trusted relationships with patients, clinicians, and service providers to co-produce personalized actions and goals that improve health and wellbeing.
Main Responsibilities
- Provide personalized information, advice, and support to patients, taking into account their needs, circumstances, and preferences.
- Develop strong relationships with patients, clinicians, and service providers to facilitate referrals and support.
- Support the development of new community groups to address gaps in services and promote community engagement.
- Co-produce personalized actions and goals with patients to track progress and improve health outcomes.
Person Specification
We are looking for an individual with a strong understanding of the wider determinants of health, including social, economic, and environmental factors. You should have excellent communication and interpersonal skills, with the ability to work independently and as part of a team. A relevant academic qualification and experience in a similar role would be advantageous.
