Job Summary

Hertfordshire Community NHS Trust is seeking a dynamic and motivated Band 6 Caseload Manager to join our Stort Valley and Villages Integrated Community Team.

Main Responsibilities
  • Provide support to a multidisciplinary team of healthcare professionals in delivering clinical health and social care services.
  • Set and maintain standards of care, deploying, coordinating, and supervising the nursing team as required.
  • Be responsible for the assessment, planning, implementation, and evaluation of individualized nursing care for patients living in the community.
  • Maintain accurate patient and other records.
  • Participate in the long-term management of patients within the locality.
  • Have continued responsibility for a defined caseload.

Requirements
  • Registered Nurse with a Specialist Nursing Practice (District Nursing) Degree or equivalent.
  • Mentor/Preceptor/998 or equivalent.
  • Nurse Prescribing or will undertake training.
  • Evidence of personal development.
  • Evidence of experience at Band 5 Community Nurse level.
  • Management of a team.
  • Management of change within a complex environment.
  • Experience of teaching others.
