Position Overview

We are seeking an Assistant Ward Manager for a permanent part-time role (30 hours per week) that requires flexibility to work throughout the week, including weekends.

The Jean Heyes Reablement Unit is a 24-bed community facility catering to both male and female patients, focusing on the rehabilitation of individuals who have been medically stabilized. This unit assists patients who are ready for discharge from hospital but need a period of bed-based rehabilitation led by nursing and therapy professionals. Our goal is to help individuals achieve specific objectives that facilitate a safe return to their homes or the community.

We are looking for candidates with relevant qualifications and experience. The available positions present exciting opportunities to concentrate on the rehabilitation of adults, deliver hands-on care, enhance quality, and provide professional clinical leadership within the unit.

An Enhanced DBS check with Adults' Barred List verification will be conducted as part of the pre-employment process.

Applicants must possess extensive post-qualification experience within the NHS.

Main Responsibilities

  • Ensure that a high standard of care is maintained by the nursing team within the Jean Heyes Reablement Unit through evidence-based practices.

Job Duties

Key Responsibilities

  • Act as a clinical role model to ensure high standards are integrated into clinical practice, delegating nursing care and interventions to team members based on their knowledge levels.
  • Collaborate with the Team Leader to proactively support clinical nursing advancements within the unit.
  • Coordinate the nursing team and prioritize workloads in response to the service's varying demands.
  • In the absence of the Team Leader, manage the duty roster, negotiating with team members to ensure adequate coverage.
  • Receive daily referrals and take appropriate actions to meet patient needs safely and promptly.
  • Maintain effective communication, both verbal and written, to ensure clarity and understanding.
  • Stay informed about organizational issues through team meetings.
  • Work closely with hospital services, other intermediate care services, social services, and the voluntary sector to ensure seamless service delivery.
  • Receive referrals from various sources and maintain high standards of patient care while identifying training needs for junior team members.
  • Facilitate a learning environment for team members and oversee student placements.
  • Be accountable for educating patients and their families, enabling informed decisions regarding their care.
  • Participate in the formal and informal training of staff, demonstrating nursing care procedures as necessary.
  • Ensure team members maintain current knowledge and skill development.
  • Incorporate health promotion principles into practice to empower patients through information and involvement.
  • Recognize factors contributing to adult abuse and refer to social services as needed.
  • Communicate effectively with patients and families to ensure their involvement and consent in care planning.
  • Act as a clinical role model for nursing staff and assistants.
  • Participate in the recruitment and selection process as required.
  • Assist in maintaining accurate data for service monitoring and development.
  • Ensure timely submission of statistical data in accordance with Trust policies.
  • Engage in a multidisciplinary approach to service delivery, fostering close collaboration.
  • Develop skills in communicating with patients who have barriers to understanding.
  • Provide support to patients and families during sensitive discussions, including those involving terminally ill patients.
  • Deliver care in a manner that reflects the team's philosophy and promotes patient independence.
  • Assess nursing care needs and develop, implement, and evaluate care plans in collaboration with other health and social care professionals.
  • Be responsible for the admission and discharge of patients, ensuring responsiveness to their needs.
  • Prioritize caseloads and respond to calls accordingly.
  • Follow individual care and treatment plans under indirect supervision.
  • Assist qualified therapists in preparing treatment materials and equipment.
  • Deliver care as prescribed by qualified staff, promoting patient independence.

Communication and Key Relationships

  • Ensure documentation complies with Trust and departmental requirements.
  • Adhere to care plans and report any changes in patient needs to qualified staff immediately.
  • Attend departmental meetings to receive updates on strategic plans and organizational objectives.
  • Record interventions accurately in patient records, following Trust guidelines.
  • Contribute to multidisciplinary collaboration by attending meetings and providing feedback on patient progress.
  • Support patients, families, and carers by respecting their privacy, dignity, culture, and human rights.

Financial Responsibilities

  • Contribute to the effective use of resources.
  • Assist in maintaining stock and equipment requisitioning efficiently.
  • Request and authorize additional staff bookings to cover absences in agreement with the team leader.

Human Resources Responsibilities

  • Participate in annual performance reviews and development planning.

Health & Safety Responsibilities

In compliance with the Health & Safety at Work Act 1974, the post holder is required to actively manage risk in all actions, including conducting risk assessments and reporting incidents.

Teaching Responsibilities

  • Engage in formal and informal teaching of staff, demonstrating nursing care procedures as appropriate.
  • Attend supervision sessions with allocated mentors to ensure competencies are maintained.
