Staff Nurse

We are seeking a highly skilled and experienced Registered Nurse to join our team in the Discharge Lounge at Darlington Memorial Hospital or University Hospital of North Durham. As a key member of our team, you will play a vital role in ensuring the smooth and efficient discharge of patients from our hospitals.
Key Responsibilities- Assess patients' needs and develop, implement, and evaluate programmes of care.
- Organise the delivery of nursing care, deploying staff effectively.
- Communicate with the Wards, Doctors, Pharmacy, Relatives, Carers, and Transport and Patient Flow Team.
- Supervise discharge lounge Health Care Assistants, liaise with the Discharge management team/ward coordinators and relatives.
- Be responsible for patient care during their time in the discharge lounge and efficient coordination of booking patient transport and checking prescribed medication in readiness for safe discharge.
- Require effective communication skills to create and build a good network of relationships with all wards and departments.
- Have a nominated base and be expected to travel and work across both sites, if required, to meet service need during times of pressure.
County Durham & Darlington NHS Foundation Trust provides hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
