Job Summary

We are seeking a skilled Registered Nurse to join our Discharge Lounge team at County Durham & Darlington NHS Foundation Trust. As a Band 5 Staff Nurse, you will play a vital role in ensuring the smooth discharge of patients from our hospitals.

Main Responsibilities

Assess patients' needs and develop, implement, and evaluate programmes of care.

Organise the delivery of nursing care, deploying staff effectively.

Communicate changes to patient information and treatment plans to team members, both verbally and in writing.

Adhere to NMC standards of record keeping.

Assist the Ward Sister in setting, monitoring, and maintaining standards of care.

Participate in nursing audit and service improvement activities, Essence of Care.

Be aware of resource constraints and the importance of working within budget.

Provide advice and act as a role model to other members of the team.

Be proactive in managing risk and maintaining patient safety.

Supervise the work of nursing support staff, ensuring tasks have been delegated appropriately.

Contribute to the day-to-day management of the Directorate, cooperating with colleagues to provide an appropriate level of nursing service and address bed management issues.

Participate in education and training of junior staff.

Involving patients/carers in the planning and delivery of care.

Participate in clinical supervision.

Assist in resolving conflict and defusing situations where appropriate.

Promote a positive and professional image of nursing at all times.

Requirements

  • Experience of working within a busy ward environment.
  • NMC Registered.
  • Registered Nurse.
  • Good interpersonal and communication skills.
  • Ability to work using own initiative.
  • Plan own workload and work effectively within a multi-disciplinary team.
  • Organisational abilities.
  • Computer skills.

Desirable

  • Team leader experience within the ward setting.
  • Understanding of the multidisciplinary discharge process.
  • Understanding and experience of nurse-led Discharge.

Working Arrangements

Flexibility regarding shift patterns and sites in accordance with service needs.

Good time keeping/attendance.

Interest in Discharge Planning.
