About the Role

We are seeking an experienced Mental Health Social Worker to join our fully integrated multidisciplinary Community Mental Health Team. As an AMHP practitioner, you will work closely with our multidisciplinary team to provide high-quality care and support to vulnerable individuals in our community.

Key Responsibilities

  • Conduct comprehensive assessments and develop personalized care plans to promote recovery and empowerment
  • Work collaboratively with service users, their carers, and families to achieve positive outcomes
  • Develop and maintain effective relationships with partner agencies and organizations to ensure seamless care coordination
  • Participate in practice education and supervision to enhance professional development and skills

Requirements
