About the Role

We are seeking a highly skilled and dedicated Mental Health Social Worker to join our team at Inclusion, a leading provider of specialist services across England. As a key member of our Integrated Mental Health and Substance Misuse Service, you will play a vital role in supporting individuals with complex mental health needs within a prison setting.

Key Responsibilities
  • Assess, plan, implement, and evaluate evidence-based interventions for service users.
  • Manage a caseload of service users, practicing within the framework of the multi-disciplinary team and Care Programme Approach.
  • Provide a care coordinator role with a specialist contribution, focusing on release planning and rehabilitation.
  • Work collaboratively with individuals from initial assessment through to discharge, including transferring care to other providers.
  • Support service users to live decent and purposeful lives as members of the community.
Requirements
  • Professional Social Work qualification, DipSW, CQSW, CSS, or Foreign qualification certified by the Health and Care Professions Council.
  • Registration with the Health and Care Professions Council at all times.
  • Commitment to ongoing personal and professional development.
  • Experience of working with service users with complex mental health problems.
  • Desirable: Current Approved Mental Health Professional status or commitment to undergo appropriate training.
