**Job Summary:**
We are seeking a skilled and dedicated Early Help Open to Review Coordinator to join our 0-25 social care service. As a key member of our team, you will work collaboratively with the 0-18 social care team to provide high-quality support to disabled children and young people.
**Key Responsibilities:**
- Conduct home visits to assess the needs of children and families
- Chair review meetings to discuss and agree on support plans
- Develop and maintain relationships with service users, agencies, and providers
- Monitor and review plans to ensure the best possible outcomes for children and families
- Collaborate with colleagues to share information and best practices
**Requirements:**
- NVQ Level 3 or equivalent qualification in services for children and young people
- Experience working in a statutory setting with children and families
- Excellent communication and interpersonal skills
- Ability to work effectively in a team and meet deadlines
- Knowledge of childcare legislation and risk assessment
