Job Title: Band 6 Diabetes Specialist Nurse

We are seeking a highly skilled and experienced Band 6 Diabetes Specialist Nurse to join our community Diabetes Service within Birmingham Community Healthcare NHS Trust Foundation.

The successful candidate will have a strong background in diabetes nursing and be able to demonstrate an in-depth knowledge of diabetes management. They will be responsible for providing assessment and advice on appropriate diabetes management, empowering patients with the skills and knowledge necessary to attain independence in self-management.

Main Responsibilities:
  • Provide clinical assessments and develop care plans for patients with diabetes
  • Work closely with local GP surgeries, secondary care diabetes colleagues, and community inter-disciplinary teams to deliver high-quality diabetes care
  • Deliver structured education to patients and healthcare professionals
  • Contribute to the development of enhanced diabetes leadership and management skills
Requirements:
  • Nursing degree in a relevant subject
  • Evidence of ongoing professional development
  • Teaching qualification
  • Independent Nurse Prescriber or willingness to undertake
Desirable Skills:
  • Excellent planning and organisation skills
  • Ability to work autonomously and use initiative
  • Experience of delivering community-based diabetes care
