We are looking for someone who embodies our Trust's vision and values, is passionate about mental health, and has a core profession in Mental Health Nursing. Ideally, you will have experience working independently in a mental health setting.
Main Responsibilities- Assess care needs and manage a defined caseload within the Cornwall Partnership NHS Foundation Trust Mental Health Service.
- Develop and implement programs of care, utilizing the care coordination system.
- Develop the interface between primary and secondary care services to offer focused and coordinated packages of care.
- Have knowledge of the CPA process, legislation around mental health, and The Mental Health Act.
- Work flexibly as part of a team to deliver innovative health and social care.
We are an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Person Specification- Current professional registration - NMC/HCPC
- Evidence of post-basic training
- Extensive experience of working with mental health patients
