Position Overview

The Enhanced Access model of care at K2 Healthcare Ltd aims to provide routine general practice consultations at the patient's registered GP practice or other local NHS facilities during evenings and weekends.

This initiative is supported by a diverse team of healthcare professionals, including General Practitioners, nurses, and various specialists, who deliver a range of services tailored to clinical requirements. These services encompass both in-person and remote consultations via telephone or online platforms.

This national initiative is designed to enhance access to routine appointments during non-traditional hours, addressing local demand while ensuring effective access to urgent care and out-of-hours services.

Furthermore, this model serves as a proactive solution for patients who might otherwise seek care in emergency departments.

Ultimately, this approach aims to improve primary care accessibility across the region, fostering collaboration among practices and enhancing engagement with other professionals in the health and social care sectors.

Key Responsibilities

As a Healthcare Practitioner within the Enhanced Access framework, you will play a vital role in delivering essential healthcare services to patients registered at various GP practices.

During the Enhanced Access hours, the range of services provided may be more limited compared to standard operating hours. Examples of services include, but are not limited to:

  • Blood Pressure Assessments - 15 minutes
  • Orthostatic Blood Pressure Evaluations - 15 minutes
  • Medication Reviews - 15 minutes
  • Laboratory Tests - 15 minutes (patients must bring a labeled specimen bag)
  • Height and Weight Measurements - 15 minutes
  • Wound Dressings - 30 minutes (patients must provide dressings)
  • Hypertension Monitoring - 30 minutes
  • Suture Removal - 15 minutes
  • NHS Health Checks - 30 minutes
  • Swab Tests - 15 minutes

Core Duties

The following responsibilities are essential for the Healthcare Practitioner role, with the understanding that additional tasks may arise based on workload and staffing needs:

  • Assess patient needs to ensure appropriate healthcare delivery, utilizing evidence-based practices.
  • Develop and implement health promotion initiatives and wellness programs.
  • Formulate and evaluate individualized treatment plans for patients with chronic conditions.
  • Identify and support patients at risk of developing long-term health issues.
  • Diagnose and manage both acute and chronic health conditions as necessary.
  • Provide guidance on prescribed and over-the-counter medications, including side effects and dosages.
  • Request laboratory services and manage pathology results as needed.
  • Maintain accurate clinical documentation in compliance with relevant legislation.
  • Utilize read codes effectively for patient records.
  • Ensure adherence to the organization's chaperone policy.
  • Prioritize health concerns and intervene appropriately.
  • Recognize and refer patients with mental health needs.
  • Support patients in the proper use of their medications.
  • Collaborate with external services to ensure comprehensive patient support.
  • Delegate clinical responsibilities safely and appropriately.
  • Assist the clinical team with safeguarding matters in line with local and national policies.
  • Deliver opportunistic health promotion when suitable.
  • Conduct health screenings and provide guidance upon patient arrival.
