About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Northern Healthcare. As a Support Worker, you will play a vital role in supporting individuals with mental health diagnoses, learning disabilities, or autism to live fulfilling and independent lives.

Key Responsibilities
  • Provide person-centered support to service users, enabling them to manage daily activities and live as independently as possible.
  • Support service users with budgeting, domestic tasks, and meal preparation, promoting their autonomy and self-sufficiency.
  • Accompany service users on grocery shopping trips and attend routine appointments, ensuring their physical and emotional needs are met.
  • Administer prescribed medication and provide emotional support to service users, promoting their mental well-being.
  • Encourage service users to engage in community activities, develop positive relationships, and participate in leisure activities, promoting their social and emotional growth.
  • Regularly review support plans, ensuring they are tailored to meet the individual needs and aspirations of service users.
  • Collaborate with colleagues to prioritize outcome-focused support, ensuring a seamless and effective service delivery.
  • Maintain accurate records, reporting in a factual and person-centered approach, and escalate concerns or issues in a timely manner.
