NELFT NHS Foundation Trust is seeking a dedicated and experienced Health Visitor to join our team.
As a vital member of our multidisciplinary team, you will play a crucial role in promoting the health and wellbeing of children under five years old. You will work independently and collaboratively with families, other agencies, and healthcare professionals to deliver comprehensive child health programs and interventions.
Key Responsibilities:- Conduct thorough assessments of children's health needs, actively involving clients and caregivers in the process.
- Monitor the health and development of children under five years old, ensuring their well-being is prioritized.
- Take proactive steps to protect vulnerable children and adults within our community.
- Serve as a trusted resource for other agencies, providing expert guidance on child health matters.
- Effectively manage a team-based caseload, ensuring timely and efficient delivery of services.
- Implement evidence-based Child Health Promotion programs to enhance the health outcomes of children and families.
To be successful in this role, you will need:
- A Registered Health Visitor Specialist Practice Qualification.
- Proven experience as a Health Visitor within multi-professional teams.
