Job Summary:

The postholder will be responsible for overseeing and providing final sign-off of clinical plans to support veterans' ongoing physical health, presenting these plans at a virtual Multi-Disciplinary Team (MDT) for agreement.

This role will involve liaising with General Practitioners, hospital consultants, military charities, and other stakeholders to ensure that more complex referrals are supported with a holistic overview, coordination of other stakeholders, and liaison with the Ministry of Defence (MOD) and Personnel Recovery Units (PRUs) for personnel leaving the Armed Forces who meet the criteria of the service.

This is a hybrid post, with the expectation that the postholder will be based at their home with occasional travel into London or other parts of England.

  • Key Responsibilities:
  • Developing key partnerships to ensure effective whole system working through effective communication and networking with other disciplines and by working across professional boundaries.
  • Coordinating clinical plans to support veterans' ongoing physical health.
  • Liaising with stakeholders to ensure effective referral processes.
  • Providing final sign-off of clinical plans.
  • Presenting plans at virtual MDTs for agreement.
