Family Hub Practitioner Role

As a Family Hub Practitioner with Kent County Council, you will play a vital role in supporting children and young people, aged 0-19 (SEND 25), and their families. Our Family Hubs offer a range of early help services, including midwifery, health visiting, parenting support, and activities for children and young people.

Key Responsibilities
  • Provide 1:1 and group support to children and their families in a variety of outreach settings, including home visiting.
  • Work closely with integrated children's services teams and multi-agency partners to provide support for vulnerable children and their families.
  • Facilitate and support peer groups, promoting the use of the Family Hub Digital and Virtual offer and other routes of advice and guidance.
  • Provide expert guidance to families on baby and child development, enabling parents to support their child's growth and achievement in health and education outcomes.
Requirements

You will be expected to work flexibly across the SEND 25 age range and may be required to support work across the wider age range where needs arise in the district. This post will be expected to work flexibly, including on occasions, evenings and weekends, and will be coordinated by management as required.

This post is subject to an Enhanced Disclosure Application to the Disclosure and Barring Service. The Council has a statutory duty to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements.
