Job Summary

The Care Coordinator - Learning Disabilities and Autism plays a vital role in the Transforming Care Programme, working towards improving health outcomes for individuals with learning disabilities and autism.

Key Responsibilities

  • Providing expert advice and guidance to healthcare professionals and social workers
  • Coordinating community services to enable individuals to remain in their community
  • Facilitating timely discharges from inpatient facilities
  • Working across services to ensure coordinated care and timely responses
  • Chairing Care and Treatment Reviews as needed

Requirements

  • Experience working with individuals with complex needs
  • Ability to make independent decisions and solve problems effectively
  • Strong communication and interpersonal skills
  • Valid driving license and ability to travel within Essex and beyond
