Care Coordinator

Job Summary
The Care Coordinator - Learning Disabilities and Autism plays a vital role in the Transforming Care Programme, working towards improving health outcomes for individuals with learning disabilities and autism.
Key Responsibilities
- Providing expert advice and guidance to healthcare professionals and social workers
- Coordinating community services to enable individuals to remain in their community
- Facilitating timely discharges from inpatient facilities
- Working across services to ensure coordinated care and timely responses
- Chairing Care and Treatment Reviews as needed
Requirements
- Experience working with individuals with complex needs
- Ability to make independent decisions and solve problems effectively
- Strong communication and interpersonal skills
- Valid driving license and ability to travel within Essex and beyond
