We are seeking a highly skilled and experienced Clinical Specialist Frailty Occupational Therapist to join our Palliative Care Team at University Hospitals Sussex NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing expert care and assessment to patients with progressing life-limiting illnesses.
Key Responsibilities- Provide clinical excellence, leadership, and innovation in palliative care, utilizing highly developed specialist knowledge.
- Act as a nurse adviser to patients, carers, and staff, providing specialist information and support for those with palliative care/end-of-life needs within the hospital setting.
- Undertake holistic assessments of patients' and carers' needs, ensuring that they are recorded and communicated to the appropriate individuals.
- Promote an optimal environment for the delivery of care for patients within the hospital setting, in partnership with other healthcare professionals.
- Facilitate the discharge of patients in accordance with rapid discharge guidelines and their preferred place of care and death.
- Facilitate discussions around advance care planning and medical contingency plans, where possible and appropriate.
- Provide data and reports that meet the required key performance indicators for the service in a timely manner.
- Offer education and training to members of nursing and medical teams within the trust.
- Support junior members of the hospital palliative care team.
- NMC Registered Adult Nurse.
- Educated to Degree level or professional knowledge acquired through degree/diploma supplemented by specialist training, experience, or masters modules.
- Specialist qualification in palliative care or specialist-related qualification experience or experience.
- Non-medical prescribing qualification.
- Physical assessment qualification.
- Psychological Assessment training Level 2.
- Communication skills training.
- Proven extensive palliative care experience at a band 6 in a hospital setting.
- Experience in supporting patients and carers with complex physical and psychological needs.
- Supporting/teaching others caring for palliative and end-of-life patients.
