Key Responsibilities- Provide clinical triage and assessment to identify individuals with complex needs and develop personalized care plans.
- Work collaboratively with community teams, GPs, and other healthcare professionals to ensure effective care coordination and delivery.
- Develop and implement proactive care strategies to prevent hospital admissions and improve patient outcomes.
- Provide education and support to individuals, families, and carers to empower them to manage their health and wellbeing.
- Conduct regular reviews and assessments to ensure care plans are up-to-date and effective.
- Collaborate with other healthcare professionals to develop and implement new care models and services.
