We are currently seeking a dedicated and compassionate nurse to join our exceptional team at Saint Francis Hospice. This role is ideal for individuals who possess strong communication skills and a genuine passion for providing high-quality palliative care. The successful candidate will work collaboratively within a supportive and dynamic nursing team, with occasional responsibilities during night shifts.
Key Responsibilities
As a Palliative Care Nurse, your primary duties will include:
- Delivering comprehensive physical, emotional, and spiritual support to patients and their families.
- Collaborating with multidisciplinary teams to ensure seamless and holistic care.
- Utilizing evidence-based practices to develop and implement care plans.
- Maintaining effective communication with patients, families, and healthcare professionals.
- Monitoring and reporting any changes in patient conditions to ensure optimal symptom management.
- Accurately documenting all care provided in accordance with established guidelines.
- Participating in quality assurance initiatives and clinical audits to enhance care delivery.
- Engaging in reflective practice and professional development activities.
Team Leadership and Collaboration
In this role, you will also:
- Assist in organizing the nursing team in the absence of senior staff.
- Ensure adequate staffing levels to maintain high standards of care.
- Foster effective communication across all departments to support integrated service delivery.
Professional Development
