We are seeking a skilled and compassionate Nurse Assessor to join our Cornwall Onward Care Team at Derriford Hospital. This exciting opportunity will enable you to support patient flow from an acute hospital setting, working collaboratively with an experienced team.
The aim is to achieve a seamless transfer of care to another setting, ensuring patients only stay in hospital for the time required to complete their acute treatment. Your role will involve:
- Acting as a link between agencies and departments, providing efficient and effective communication to expedite progress towards onward care.
- Collaborating with clinical teams, health and social care agencies, and the Integrated Transfer of Care Hub to plan and manage the flow through the Acute trusts.
- Contributing to follow-up assessments, care planning, identification of resources, and service delivery to facilitate the timely discharge of patients.
- Working flexibly and proactively in response to the needs of the service, across organisational boundaries.

Person Specification
Education/ Qualification
- Current professional registration - NMC
- Professional qualification - RN, RMN
- Post graduate learning - Alert or Long term conditions
- Post registration experience

- Excellent communication skills to deal with people who may be upset, angry, aggressive, and have communication difficulties themselves
- Ability to work on own initiative as role may involve Lone Working
- Ability to cope with changing workload/pressures in an environment which can be physically, emotionally, and mentally demanding
