About the Role

We are seeking a highly skilled and motivated Mental Health Practitioner to join our EIP Team at Lincolnshire Partnership NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to individuals with severe mental health problems.

The successful candidate will have a strong background in mental health nursing, occupational therapy, or social work, with a focus on working with individuals with complex mental health presentations involving psychosis. You will be responsible for assessing, planning, and implementing evidence-based care to a defined group of service users and their carers.

Key Responsibilities:

  • Provide high-standard clinical care, ensuring safe and effective assessment, treatment, and comprehensive discharge planning.
  • Delegate aspects of patient treatment plans to other team members, while maintaining overall responsibility for patient care.
  • Ensure the clinical caseload and practice of nursing members of the EIP Team meet the highest standards of clinical care.
  • Work collaboratively with individuals to assess their mental health, recognize mental illness, and identify related needs and circumstances.
  • Plan, implement, review, and improve interventions to meet individuals' identified needs and manage inherent risk.
  • Enable individuals to develop independent living skills and live in the community.

Requirements:

  • RMN, OT, or Social Worker core professional background education to diploma/degree level.
  • Professional registration with NMC, HCPC, or other relevant regulator.
  • Mentorship Module or equivalent (degree level).
  • Evidence of specialized continued professional training (degree level) in clinical practice.
  • Management training.
  • Leadership training.
