We are seeking an experienced General Practice Pharmacist to join our team at NHS. As a key member of our multidisciplinary team, you will provide clinical pharmacy leadership and support to our GP practices, working closely with healthcare professionals to improve patient care and outcomes.
Key Responsibilities- Provide clinical pharmacy leadership and support to GP practices, including structured medication reviews, chronic disease management, and medicines optimization.
- Lead, train, and coordinate the work of less experienced pharmacy professionals to manage repeat prescribing systems, deal with acute prescription requests, and medicines reconciliation.
- Provide expertise in clinical medicines advice, addressing both public and social care needs of patients in GP practices.
- Support practice staff with prescription and medication queries, ensuring integration with community and hospital pharmacy.
- Minimum of 5 years post-registration experience with at least 2 years in General Practice.
- Certificate in Non-medical prescribing.
- Experience of using prescribing qualification in General Practice.
- Evidence of further education in clinical pharmacy, e.g., Completion of NHS Pathway Training for Practice Pharmacists or a Clinical Pharmacy Diploma.
