Family Practitioner Opportunity at Acorns Children's Hospice

Acorns Children's Hospice is seeking a dedicated Family Practitioner to join our Family Services Team in Our Walsall Hospice. As a Family Practitioner, you will play a vital role in providing holistic psychosocial, bereavement, and end-of-life support to families in their homes, hospice, or other suitable environments.

Key Responsibilities:
  • Provide ongoing support to families, maintaining their emotional and psychological well-being.
  • Develop and maintain professional relationships with families, healthcare professionals, and other stakeholders.
  • Contribute to the development and implementation of family-centered care plans.
  • Maintain accurate records and reports, ensuring compliance with regulatory requirements.
  • Participate in ongoing professional development, staying up-to-date with best practices and research in the field.
Requirements:
  • Relevant qualifications and experience in family-centered social care or nursing.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse families and professionals.
  • Strong assessment and problem-solving skills, with the ability to think critically and creatively.
  • Ability to work independently and as part of a team, with flexibility to adapt to changing priorities and circumstances.
  • Valid driver's license and ability to travel independently across the region.
