About the Role

Hallmark Luxury Care Homes is seeking a highly skilled and compassionate Dementia Care Manager to lead our dementia community and drive our relationship-centred dementia care strategy.

Key Responsibilities:

  • Lead and manage a dedicated team of carers to deliver exceptional dementia care
  • Develop and implement dementia care plans that meet the unique needs of each resident
  • Collaborate with the General Manager and other department heads to ensure seamless care delivery
  • Ensure the dementia community meets all CQC and Hallmark quality and compliance standards
  • Build strong relationships with local community groups and partners in dementia care
  • Support family members and friends of residents with dementia

Requirements:

  • Passionate about dementia care and committed to delivering exceptional care
  • Strong leadership and management skills
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with a multidisciplinary team
  • Knowledge of dementia care principles and best practices
