Job Title: Adults Community Physiotherapist

We are seeking a skilled and dedicated Physiotherapist to join our team in Central and North West London NHS Foundation Trust. As a Physiotherapist in our Community Health Service, you will play a vital role in providing high-quality rehabilitation care to patients in their homes and community settings.

About the Role

This is an exciting opportunity to work in a dynamic and interdisciplinary team, providing rehabilitation services to patients with a wide range of conditions, including post-orthopaedic surgery, general surgery, critical care, frailty, stroke, COPD, dementia, MS, musculoskeletal, and palliative needs.

You will be responsible for assessing, planning, and delivering individualized treatment plans, working closely with patients, carers, and other healthcare professionals to promote independence, prevent unnecessary hospital admissions, and reduce length of stay.

As a Physiotherapist in our Community Health Service, you will have the opportunity to develop your clinical skills, work in a variety of settings, and contribute to the development of new specialist clinical services.

Key Responsibilities
  • Assess and plan individualized treatment plans for patients with complex needs
  • Deliver high-quality rehabilitation care in community settings, including patients' homes, community venues, and hospital
  • Work collaboratively with patients, carers, and other healthcare professionals to promote independence and prevent unnecessary hospital admissions
  • Develop and maintain a high-quality, efficient, and effective therapy service, aligned with the philosophical values of CNWL
  • Manage a defined caseload, using evidence-based and patient-centered principles to assess, plan, and deliver individual treatment plans
  • Participate in staff supervision, training, and completion of competencies, and observe practice to develop academic learning, clinical educator, and vocational skills
Person Specification
  • Degree qualification in Physiotherapy
  • HCPC Registration
  • Experience of working in an NHS setting, preferably in a community or rehabilitation setting
  • Ability to drive own car for work
  • Evidence of continuing professional development
  • Excellent written and IT skills
