Stroke Rehabilitation Physiotherapist

We are seeking a highly skilled and motivated Physiotherapist to join our Early Stroke Rehabilitation Team (ESRT) on a fixed-term basis. As a key member of our team, you will work closely with a multidisciplinary team to provide high-quality patient-centered care to individuals who have experienced a stroke.
Main Responsibilities- Conduct comprehensive assessments and develop individualized treatment plans for patients with stroke in a community setting.
- Work collaboratively with the ESRT team, including physiotherapists, occupational therapists, speech and language therapists, and clinical neuropsychologists, to provide holistic and goal-oriented rehabilitation.
- Implement strategies to empower patients and their families to take an active role in their rehabilitation and long-term management of their condition.
- Participate in multidisciplinary meetings and contribute to patient discussions, treatment plans, and discharge planning.

Requirements- UK registration with the Health and Care Professions Council (HCPC)
- Physiotherapy degree or equivalent professional qualification
- Broad experience working as a physiotherapist in the NHS, preferably in a community or acute setting
- Experience working with neurological conditions and/or stroke in a community or acute setting
- Excellent communication and interpersonal skills
