In this position, you will be responsible for managing and delivering direct nursing services to participants in the PACE (Program of All-Inclusive Care for the Elderly) program across various environments, including clinics, homes, and virtually.
This role is essential to the Interdisciplinary Team (IDT) and requires collaboration to conduct assessments and develop participant care strategies. It offers a chance to engage in a culture that prioritizes quality outcomes, teamwork, and compassion in addressing the unique needs of each participant.
Key Responsibilities:
- Contribute significantly to the Interdisciplinary Team (IDT) by participating in the creation of comprehensive care plans, conducting patient evaluations, delegating tasks, and managing service determination requests.
- Oversee personalized care for a designated group of participants in partnership with a medical provider, serving as the primary contact for the IDT to ensure adherence to each participant's care plan.
- Provide care in various settings where participants reside (home, clinic, virtual, assisted living, skilled nursing facility) addressing needs such as basic primary care, medication management, wound care, and other clinical responsibilities within your scope of practice.
- Conduct nursing evaluations that encompass physical, psychosocial, and behavioral aspects.
- Recognize emergency situations and independently initiate appropriate interventions within your professional scope.
- Educate participants, caregivers, and staff on tailoring and implementing care plans while responding to specific participant circumstances.
- Monitor and evaluate therapeutic interventions, suggesting proactive adjustments to improve health and well-being.
Schedule and Shift Details:
Full-time position requiring 40 hours per week, Monday through Friday, with occasional on-call responsibilities.
